ABSTRACT

To evaluate the value of endoscopic retrograde cholangiopancreatography(ERCP)and SpyGlass in the diagnosis of intraductal papillary mucinous neoplasm of the bile duct (IPMN-B). Data of patients who underwent ERCP and SpyGlass in Hangzhou First People′s Hospital from January 2016 to December 2019 were analyzed. ERCP and SpyGlass features, complications, clinicopathologic characteristics and prognosis were retrospectively analyzed.A total of 9 patients (5 benign lesions and 4 malignant lesions) were included.ERCP was successfully performed in 9 cases, while SpyGlass was technically successful in 8 cases. Endoscopy showed mucus outflow from the papilla in 5 cases, and the mucus was removed by the balloon of ERCP in 8 cases.ERCP showed bile duct diffuse dilatation and filling defects in all patients. SpyGlass found the mucus in the bile duct in all patients. SpyGlass showed lesion mucosa were fish-egg like without vascular images (Ⅱtype, 3 cases), fish-egg like with vascular images (Ⅲ type, 1 case), villous (Ⅳtype, 4 cases). SpyGlass defined extent of the lesion in 8 cases. SpyGlass found that the lesion involved the intra and extrahepatic bile ducts in one case. Therefore, liver transplantation was recommended to avoid surgical exploration. One type Ⅲ lesion underwent a direct biopsy. The pathology showed moderate dysplasia, which was consistent with the postoperative pathology. No complication occurred. ERCP combined with SpyGlass could clarify the scope of IPMN-B and provide basis for surgical options, which is safe and effective in IPMN-B diagnosis.

ABSTRACT

OBJECTIVE@#To assess the changes in the coagulation profiles of patients with chronic kidney disease (CKD) using thromboelastography (TEG) and identify the risk factors of hypercoagulation in CKD patients.@*METHODS@#A total of 128 patients with CKD admitted in Hunan Provincial People's Hospital between August, 2018 and May, 2019 were recruited. The results of conventional coagulation test and TEG were compared between patients with CKD and 21 healthy control adults. The patients with CKD were divided into hypercoagulation group with a maximum amplitude (MA) > 68 mm (=66) and non-hypercoagulation group (MA≤68 mm, =62). The laboratory indicators were compared between the groups, and the factors affecting the hypercoagulable state in patients with CKD were analyzed.@*RESULTS@#The levels of fibrinogen and D-Dimer increased significantly in patients with CKD at different stages as compared with the control subjects ( < 0.05). In the patients with CKD, the reaction time and K time decreased while MA, α-angle and coagulation index increased significantly in patients in stage 3-4 and those in stage 5 either with or without hemodialysis compared with the control group ( < 0.05). The estimated glomerular filtration rate (eGFR), percentage of patients with diabetes mellitus, history of stroke, percentage of neutrophils, neutrophil-lymphocyte ratio, red blood cell count, hemoglobin levels, platelet count, serum creatinine, serum cystatin-C, serum albumin, and lipoprotein (a) all differed significantly between hypercoagulation group and non-hypercoagulation group ( < 0.05). The eGFR, platelet count and hemoglobin levels were identified as independent factors affecting hypercoagulability in patients with CKD ( < 0.05).@*CONCLUSIONS@#s The hypercoagulable state of patients with CKD worsens gradually with the disease progression, and eGFR, platelet count and hemoglobin levels are all risk factors for the hypercoagulable state in patients with CKD.

ABSTRACT

Objective:To investigate the diagnostic value of SpyGlass endoscopy system (short for SpyGlass) for indeterminate bile duct stricture.Methods:Data of patients who underwent SpyGlass examination for indeterminate bile duct stricture at Hangzhou First People′s Hospital from September 2012 to August 2017 were reviewed. The diagnostic value of SpyGlass for indeterminate bile duct stricture was analyzed.Results:A total of 88 patients with indeterminate biliary stricture were diagnosed with SpyGlass. The procedure success rate was 97.7% (86/88). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of SpyGlass for indeterminate bile duct stricture were 98.1%(52/53), 96.9% (31/32), 97.6% (83/85), 98.1% (52/53)and 96.9% (31/32), respectively. Three patients (3.5%, 3/86) had postoperative complications and were cured by conservative treatment.Conclusion:SpyGlass is safe and effective for indeterminate biliary stricture with high sensitivity and accuracy and low incidence of complications.

ABSTRACT

Objective To investigate the characteristics of population and the changes of disease spectrum in patients treated by endoscopic retrograde cholangiopancreatography ( ERCP ) . Methods Data of 20170 patients, who underwent ERCP in the First People' s Hospital of Hangzhou from May 2004 to February 2018, were enrolled in the retrospective analysis. According to the year of diagnosis and treatment,patients were divided into 2004-2008, 2009-2013, and 2014-2018 group; according to the postoperative diagnosis of ERCP, they were divided into biliary tract diseases ( including calculus, benign stenosis, malignant stenosis, and other causes ) and pancreatic diseases ( including acute pancreatitis, chronic pancreatitis, pancreatic malignant tumor, and other causes ) group; and according to the age, they were divided into the younger age (0-18 years old), young and middle age (19-65 years old), old age (66-85 years old), and advanced age (>85 years old) group. Statistical analysis was performed in the different groups. Results Among the 20170 patients, there were 10260 males and 9910 females, with age of 62. 65± 17. 11 years. The proportion of the younger age group and the advanced age group was 1. 04%( 24/2308 ) , 1. 69% ( 127/7520 ) , 2. 39% ( 247/10342 ) , and 2. 95% ( 68/2308 ) , 4. 19%( 315/7520 ) , 6. 15% ( 636/10342 ) , respectively, in the 2004-2008, 2009-2013 and 2014-2018 subgroups, with a rising trend (P<0. 017). A total 20032 patients had a clear postoperative diagnosis, including 15618 ( 77. 97%) of biliary diseases and 4414 ( 22. 03%) of pancreatic diseases. Biliary stones accounted for the majority of biliary diseases (63. 2%, 9863/15618), and its proportion increased from 59. 9% ( 1191/1987 ) in 2004-2008 to 64. 5% ( 5118/7939 ) in 2014-2018 ( P= 0. 000 ) . Acute pancreatitis accounted for the majority of pancreatic diseases (67. 4%, 1973/4414), and its proportion increased from 52. 4% ( 162/309) in 2004-2008 to 69. 9% ( 1636/2340) in 2014-2018 ( P=0. 000) . The top 3 diseases in the younger group and the advanced age group were acute pancreatitis ( 32. 58%, 129/396) , biliary stones ( 25. 25%, 100/396 ) , chronic pancreatitis ( 22. 22%, 88/396 ) , and biliary stones (56. 46%, 568/1006), malignant biliary stenosis (12. 52%, 126/1006), benign biliary stricture ( 10. 34%, 104/1006) , respectively. Conclusion The main disease of patients receiving ERCP is biliary stone, and the proportion of acute pancreatitis is increased. The overall age of the patients is old, and the proportion of the elderly and underaged patients is gradually increasing. Biliary diseases and pancreatic diseases are the main diseases in elderly patients and younger patients, respectively.

ABSTRACT

Cortex Lycii [root back of Lycium chinense] has is a famous traditional Chinese medicine which displays several pharmacological activities including antioxidant and antidiabetic properties. We investigated the effect of the ethyl acetate fraction [QCL] of Cortex Lycii on the enzymes involved in the metabolism of carbohydrate in diabetic rat models. Streptozotocin-nicotinamide [110 and 65mg/kg body weight, respectively] was used to induce diabetes. Diabetic rats were treated with QCL [100, 200 and 400 mg/kg] and glibenclamide [600 micro g/kg] daily for six weeks. Upon the completion of treatment, fasting blood glucose [FBG], insulin, glycosylated haemoglobin [HbA1c], haemoglobin [Hb], hexokinase, glucose-6-phosphate dehydrogenase, glucose-6-phosphatase, phosphoenolpyruvate carboxykinase and fructose-1,6-bisphosphatase levels were measured by biochemical assays. Likewise, the body weight, food and water intake was monitored and measured. Diabetic rats displayed significant elevation in the blood glucose, glycosylated hemoglobin and a marked decrease in plasma insulin and hemoglobin. Furthermore, the levels of key enzymes including fructose-1, 6-bisphosphatase, and glucose-6-phosphatase phosphoenolpyruvate carboxykinase were significantly increased while the activity levels of hexokinase, glucose-6-phosphate dehydrogenase and glycogen were significantly down regulated in diabetic rats. However, treatment of diabetic rats with Cortex Lycii led to a significant reduction the FGB, food and water intake and an increase in the plasma insulin level. Treatment with Cortex Lycii also reversed the altered activity profiles of the key enzymes mentioned above in a dose dependent manner. Our results suggested that Cortex Lycii has a promising therapeutic option in the management of diabetic complications relating to glucose homeostasis and carbohydrate metabolism

ABSTRACT

Objective To study the efficacy and safety of endobiliary radiofrequency ablation (RFA) with stent placement in treatment of unresectable extrahepatic cholangiocarcinoma (EHCC).Methods The patients with unresectable EHCC in Hangzhou First People's Hospital between October 2013 and January 2015 were enrolled in a prospective, randomized, single-blind cohort study, and were assigned randomly into two groups: RFA+stent group and stent-only group.The jaundice fade time, stent patency period, overall survival rate, and postoperative adverse events were analyzed between two groups.Results Among 59 patients with non-resectable EHCC, 28 were divided into RFA+stent group, and 31 were in stent-only group.There was no statistical difference in preoperative serum total bilirubin between the two groups.Mean serum bilirubin decrease time was significantly lower in the RFA+stent group than that in the stent-only group[17.9 d(7-22 d) VS 29.9 d(10-55 d),P=0.03].The biliary patency period in RFA+stent group was significantly longer than that of stent-only group (8.9 months VS 4.5 months, P=0.02).The mean survival time in RFA+stent group was significantly longer than that of stent-only group[13.3 months(6.2-16.5 months) VS 8.6 months(4.5-11.7 months), P=0.000).Incidence of postoperative adverse events showed no statistical difference between the two groups(P=0.727).Conclusion RFA with stent placement is effective and safe as a palliative measure in the treatment of non-resectable EHCC, and it can significantly shorten the jaundice fade time, prolong the biliary patency period and overall survival, while without increase of the incidence of adverse events.

ABSTRACT

Objective To investigate the effects of penehyclidine hydrochloride injection combined with safflower injection on TLRs/NF-κB signaling pathway and T cell subsets in mononuclear cells of patients with organophosphorus pesticide poisoning. Methods 80 cases of organophosphate poisoning patients from June 2014 to June 2016 in onglu first people's hospital were selected,randomly divided into control group and observation group, the control group was treated with penehyclidine hydrochloride,observation group were treated with Honghua injection based on the treatment of control group. The effects were compared between two groups of TLRs/NF-κB in peripheral blood mononuclear cells pathway and T cell subsets in peripheral blood, and compared two groups of patients with cholinesterase activity. Results On the third,fifth and 7th day of treatment,cholinesterase activity in both groups was higher than that before treatment,the activity of cholinesterase in the observation group was significantly higher than that in the control group (P<0.05).After treatment,compared with before treatment,the levels of TLR2,TLR4 and NF-κB in the two groups were significantly decreased,and TLR2,TLR4,NF-κB levels in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05),Th17,Th17/Treg levels in the observation group were significantly lower,and lower than the control group,Treg content was significantly higher,and higher than the control group,the difference was statistically significant (P<0.05). Conclusion Penehyclidine hydrochloride injection combined with safflower injection can help to reduce Th17,Th17/Treg levels,increased Treg cell content,regulate the inflammatory response, and promote recovery of cholinesterase activity.

Objective:To optimize the ultrasound-assisted extraction process of total flavonoids in Vaccinium bracteatum Thunb. leaves. Methods:The single factor test and orthogonal test were adopted with the content of total flavonoids as the index, and the ultra-sound-assisted extraction was optimized with the concentration of solvent, extracting time, ratio of raw material to liquid and extraction times as the factors. Results:The optimal extraction conditions were 70% ethanol with 40-fold volume, extracting 2 times with 30 mi-nutes per time. Conclusion:The extraction process is stable and feasible, and all the results are useful references for the extraction process improvement.

ABSTRACT

Objective To investigate the clinical efficacy of mesalazine combined with bifico in the treatment of ulcerative co -litis (UC) and its effect on toll-like receptor 4 (TLR4) and myeloid differentiation factor 88 (MyD88).Methods A total of 86 cases of UC was randomly divided into the observation and control groups with 43 cases in each group .The observation group was given me-salazine combined bifico treatment , and the control group was given mesalazine , and eight weeks as a course of treatment .The clinical efficacy was observed .Serum TLR4 and MyD88 were tested.Adverse reaction in treatment was recorded .Results The total effective rate (95.35%) in observation group was significantly higher than that (81.40%) in control group( P 0.05 ) .Conclusions Mesalazine combined with bifico in the treatment of UC is safe and effective . The effect may be related to regulation of peripheral blood TLR 4 and MyD88 content .

ABSTRACT

In this research, we compared the visual neuron responses for LGN, A18 and PMLS of old and young cats with extracellular single-neuron recording techniques. We used firing rate vector to characterize information, and response irregularity of cells to evaluate the degeneration of visual characters. Response irregularity is characterized by means of the two coefficients of variation of firing rate vectors: Cv and Cv2. We found that there was no significant change of the response irregularity in LGN areas during the aging process from young to old cats. But in the other two areas, neurons of old cats exhibited significantly larger response irregularity than those of young cats. The result indicated that the information processing function of advanced visual cortex was impaired by aging. This result also provids a reference for the research of the other neuronal system changes during aging process.

ObjectiveTo assess the effect of PM2.5 on sympathetic innervation and the relationship with cardiac nerve growth factor in healthy rat hearts,identify the ability to the sympathetic nerve reconstruction,and explore the possible arrhythogenic mechanism of PM2.5.MethodsForty healthy SD rats were instilled into trachea with two different solutions twice per week for four weeks:control group with saline and experimental group with PM2.5 25 mg/kg.Then these rats were killed,and biatrial appendages,two ventricular anterior walls were left.Immune cytochemical staining of cardiac nerves was performed using anti-tyrosine hydroxylase antibodies and cardiac nerve growth factor was detected by western blotting.ResuitsCompared with the control group,both the density of sympathetic nerves and the expression of cardiac NGF protein in the experimental group were much higher in the left ventricular anterior wall ( P<0.01 and P<0.05) and in the right ventricular anterior wall ( P<0.01 and P<0.05).ConclusionsPM2.5 can induce regional sympathetic hyperinnervation in both ventricular anterior walls in healthy rats by stimulating the NGF protein secretion,and the sympathetic nerve reconstruction effect of PM2.5 was confirmed.

@#Objective To observe the level of social support and self-efficacy of the breast cancer patients with postoperative chemotherapy, and study the relationship between them. Methods 76 patients were investigated with Social Support Rating Scale and Strategies Used by Patients to Promote Health (SUPPH). Results The total and every dimension scores of SUPPH and Social Support decreased in the patients. The total and every dimension scores of Social Support positively correlated with every dimension scores of SUPPH (P<0.01). Conclusion The level of self-efficacy and social support may relate each other.

ABSTRACT

Objective To investigate the effect of ischemic preconditioning (IPC) on the expression of a disintegrin and metalloprotease with thrombospondin type 1 motifs (ADAMTS-1), and to study whether the application of small interfering (si)RNA specifically targeting ADAMTS-1 would help to recover IPC protection in the aged heart. Methods The 32 young (4 months) and 32 aged(24 months) male Sprague-Dawley (SD) rats were assigned randomly to IPC group (n=20) and sham operated group (n= 12) respectively. Myocardial samples from the ischemic-reperfused region were harvested for detecting the ADAMTS-1 expression. In addition, the 110 aged SD rats were assignedrandomly to ADAMTS-1 siRNA group and control group (n=55, each). The effects of ADAMTS-1siRNA transfcction on the expression of ADAMTS-1 protein, myocardial infarction survival rate,heart function and myocardial infarction size after IPC were observed.Results Twenty-four hours after IPC, the ADAMTS-1 protein expression increased significantly in iscbemic-reperfused region both in young and aged rats (P<0. 05), and the protein expression was higher in aged rats than in young rats (P<0.05). In young-IPC group, the absorbency showed ADAMTS-1 protein expression at 0 hrs and 24 hrs after IPC were 0. 05±0.01 and 0.12±0.03 by immunohistochemical staining, and were 0.68±0. 16 and 1. 17±0.21 by Western blots respectively. In aged-IPC group, the absorbency showed ADAMTS-1 protein expression at 0 hrs and 24 hrs after IPC were 0.07±0. 03 and 0.21 ±0.04 by immunohistochemical staining, and were 0. 76±0. 21 and 1. 48±0. 17 by Western blots. In the aged rats, ADAMTS-1 siRNA transfection inhibited ADAMTS-1 protein expression (0. 66±0. 19and 0.78±0.21, by Western blots at 0 hrs and 24 hrs after IPC, P>0.05), but didn't improve myocardial infarction survival rates [ADAMTS-1 siRNA group and sham operated group: 14.3% (5/35) vs. 17.1 %(6/35), P>0.05], left ventricular fractional shortening [(14.0±3.2)% vs. (13.0±2.9)%, P>0.05] and myocardial infarction size[(39.0±4.1)% vs. (38.0±5.3)%, P>0.05].Conclusions ADAMTS-1 expression induced by IPC increases significantly in aged versus in young rats. ADAMTS-1 knockdown by siRNA inhibits ADAMTS-1 protein expression but cannot recover the age-associated loss of IPC protection.

Objective To evaluate the feasibility and safety of endoscopic intraluminal closure of small colon perforation with metal clips in porcine model.Methods A 2 cm full-thickness colon perforation was made in 4 bama mini-pigs.One did not receive local closure as a control, while 3 others underwent closure with bi-and tri-arm clips, one of which received additional fibrin glue injection after closure.Necropsy was done on day 14 after the procedure, and dye leak test, peritoneal fluid bacteria culture and histological examination were performed.During the 2 week observation period, the body temperature and blood leukocytes count were recorded and compared.Results Endoscopic closure of the colon perforation was technically successful in all 3 pigs.The body temperature and blood leukocyte count of these pigs were both lower than those of the control.Necropsy showed that the perforation in control pig was not completely closed and with multiple adhesions around.Perforation complete closure and pathological healing without adhesion were observed in 2 pigs with clip closure only, while adhesion and incomplete pathological healing was detected in the other one with clip closure plus fibrinogen injection (P<0.05).Conclusion Endoscopic intraluminal closure of a 2 cm colon perforation with metal clips is feasible and safe, while further study is needed to evaluate the effect of combined local fibrinogen injection.

ABSTRACT

Objective To summarize the clinical features of the duodenal lipomas in order to explore effective diagnostic methods and appropriate treatment preoperatively. Methods The clinical features, laboratory results, endoscopic appearance, radiological and pathological data of 8 cases of duodenal lipoma were retrospectively analyzed. Results Four patients suffered with repeated melena, 3 cases with epigastric discomfort, sour regurgitation or hiccup, while 1 patient without any symptoms. Liver functions, serum lipids and tumor markers were normal in all patients. Six patients had been detected lesions by gastroscopy (2 cases missed diagnosed in the first examination) , these lesions were appeared in duodenal bulb (2 cases) or descendant duodenum (4 cases). Abdominal CT examination revealed partial duodenal wall thickening (6 cases) , partial enteric cavity narrowing (4 cases) , or low-density lesions in enteric cavity (3 cases) with CT value of -85 HU and evenly intensified when enhanced. EUS showed intensive hyperechoic lesions from submucosa, with homogeneous echo and clear margin. Biopsy under endoscopy in all patients showed chronic inflammation of mucosa, while the pathologic diagnosis was lipoma after surgical excision or endoscopic resection. Pancreaticoduodenectomy performed in 1 patient, duodenal tumorectomy in 3 patients and endoscopic resection with snare in 4 patients. Conclusion Common site of duodenal lipoma is descending part, and the clinical manifestations are non-specific. Imaging and endoscopic examination are the mainly methods to detect the lesion, while EUS is significantly valuable in diagnosing and differential diagnosing. It can be treated by partial tumorectomy or endoscopic trap resection.

ABSTRACT

<p><b>OBJECTIVE</b>In order to find the new varieties with different horticultural characters, and investigate the mutation effects of seeds of Dioscorea zingerbrensis.</p><p><b>METHOD</b>The seeds were carried by a satellite into space and recovered. The space mutation effect on the germination, seedling growth, chromosomes and rhizome diosgenin content of SP, populations of D. zingerbrensis were investigated.</p><p><b>RESULT</b>Stimulated by space environment, the seed possessed the fast germinating characteristics. Germination rate showed no change. It was also found that a few plants were aneuploid or tetraploid. Fresh rhizome weight and rhizome diosgenin content in the second year plant were remarkably higher than those of the control. However, the increasing of third year plant was slow down, and rhizome diosgenin content in the third year plant declined simultaneously.</p><p><b>CONCLUSION</b>The space environment showed stimulating effects on seed germination, fresh rhizome weight and rhizome diosgenin content.</p>

ABSTRACT

In order to investigate the interactions between first- and second-order motion perception systems, 14 adult subjects with normal or correct to normal visual acuity were recruited and divided into two groups. Then these two groups were trained, in their parafovea, to discriminate the direction of first- and second-order motion gratings respectively. The spatial frequency of the gratings used in study was fixed at 2 cycles/degree and the temporal frequency was fixed at 8 Hz during training. Contrast sensitivity of subjects in these two training groups for first- and second- order motion was measured before and after 7 days' training to estimate the effects of training. In addition, the differences between the two training groups(14 subjects) and another control group (11 subjects) were studied. The results showed that: 1) the training with first-order motion gratings can improve subjects' performance in first-order motion direction discrimination but the improvement can't be transferred to the performance in second-order motion task; 2) the training with second-order motion gratings can improve subjects' performance in both first- and second-order motion direction discrimination tasks. In conclusion, an " asymmetric transfer" occurred between the training effect of first- and second-order motion gratings. These results indicate that there are two mechanisms for perceiving first- and second-order motion respectively. However, they are not completely different from each other but only partly separated.

ABSTRACT

Objective To investigate the mechanism of ventricular dilation-induced arrhythmias by dilating isolated rat hearts. Methods Isolated rat hearts were perfused by Langerdorff method. After equilibration, 80 hearts were randomly divided into four groups as follows: (1) control group (n=20), (2) Ca2+ preconditioning (CPC) group (n=20), (3) streptomycin group (n=20), and (4) CPC + streptomycin group (n=20). A latex balloon which can be filled with fluid was anchored in the left ventricle through the left atrium and mitral valve. Epicardial ECG of the left ventricle, left ventricular pressure, coronary flow and heart rate were recorded before and during ventricular dilation by injecting fluid into the latex balloon. The rate and duration of ventricular dilation-induced arrhythmias were recorded. Results Under the same increase in ventricular end-diastolic pressure made by inflation of the balloon, the rate of arrhythmias was 100% and duration of arrhythmias was 2.56±0.46 s in the control group. Both the rates of premature ventricular beat (90 %) and ventricular tachycardia 70 % ) were high. Compared with the control group, the total rate (60 % ) of arrhythmias was lower, and duration (1.67±0.61 s ) of arrhythmias was shorter in the CPC group. Both the rates of premature ventricular beat (60%) and ventricular tachycardia (40%) were low comparatively. The rate of arrhythmias (45 %) was lower and duration ( 1.64±0.42 s)of arrhythmias was shorter, and the rates of premature ventricular beat (30 % ) or ventricular tachycardia (35 %) were lower in the streptomycin group than in the control one. The least ventricular dilation-induced arrhythmias occurred in the CPC + streptomycin group. The rate of arrhythmias (10%) was the lowest and duration (1.01±0.37s) of arrhythmias was the shortest; both the rates of premature ventricular beat (5%) and ventricular tachycardia (10%) were the lowest. Conclusions Ventricular dilation may induce arrhythmias in isolated rat hearts. Stretch-activated ion channel and the increase in [Ca2+]I are supposed to play important roles in the pathological mechanism.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of magnesium sulfate on neuron apoptosis and the expressions of caspase-3, bax and bcl-2 after cerebral ischemia-reperfusion injury.</p><p><b>METHODS</b>Thirty-six gerbils were randomly divided into three groups: Sham-operation group (So, n = 12), ischemia-reperfusion group (I-R, n = 12) and magnesium sulfate group (Ms, n = 12). The neuron apoptosis was detected by the terminal deoxynucleotidyl transferase (TdT)-mediated dUTP-fluorescence nick end labeling (TUNEL stain), and the protein expressions of caspase-3, bax and bcl-2 were detected by immunohistochemisty stain.</p><p><b>RESULTS</b>Apoptotic neurons and the expressions of caspase-3 and bax were significantly increased in I-R and Ms groups, compared with those in So group. Apoptotic neurons and the expressions of bax and caspase-3 in Ms group were significantly less than those in I-R group. There was no significant difference in the expression of bcl-2 among three groups.</p><p><b>CONCLUSIONS</b>Magnesium sulfate decreases neuron apoptosis after cerebral ischemia-reperfusion injury, which may be related with the suppressed expressions of caspase-3 and bax.</p>

ABSTRACT

Objective To explore for the effect of endoscopic treatment on biliary leakage and biliary duct stricture. Methods All patients with biliary leakage and biliary duct stricture were treated by endo scopic sphincoterotomy and endoscopic nasobiliary drainage ( ENBD) during abdominal cavity drainage. EN-BD was removed when biliary leakage healed and abdominal cavity drainage ceased for 1-2 weeks were confirmed. Plastic stents were implanted to distend the biliary duct stricture for 2-3 months. Results Twenty two patients with biliary leakage were cured 3-4 weeks after ENBD. Ten out of 13 patients implanted with plastic stent were recovered uneventfully after stent removed, and 2 patients also recovered after installation of double-stents for 3 months, while another case with calculus and stricture of left hepatic duct in spite of implantation of simple-stent suffered repeatedly from biliary tract infection. Conclusions Endoscopic therapy is the first choice in treating biliary leakage or secondary biliary duct stricture.
